The Vein Clinic recommends that patients contact their insurance carriers prior to their consultation to verify the requirements and benefits of their plans. The office staff will pre-authorize/pre-determine with your insurance company prior to scheduling your treatment, if needed. Again, the procedure will be billed to your insurance company first, and then the office will bill you for any co-pay.

Note: Blue Cross/Blue Shield of Michigan requires no pre-determination; it is a covered benefit.
